INSTALLMENT PAYMENT AGREEMENT

This Installment Payment Agreement (“Agreement”) is entered into by and between PHI BEAUTY LLC (“Service Provider” or “Business”), located at 1559 Atlantic Blvd. Neptune Beach, FL / Unit 126, and the Client with the name and address information in the following.

1. Payment Terms

1.1 The Client agrees to pay the Total Amount in monthly installments, with the first payment due at the time of signing this Agreement.

1.2 The Client authorizes PHI BEAUTY LLC to charge the following credit card for each installment on the due dates:

2. Photo Identification Requirement

2.1 The Client agrees to provide PHI BEAUTY SPA, LLC with a valid government-issued photo identification, such as a driver’s license or state ID, for verification and record-keeping purposes.

2.2 The Client acknowledges that the information will be kept confidential and securely stored, and will only be used for identity verification and enforcement of this Agreement.

3. Automatic Payments & Authorization

3.1 The Client authorizes PHI BEAUTY LLC to automatically charge the above credit card for each installment payment on the agreed due dates without further notice.

3.2 This authorization will remain in effect until the Total Amount is paid in full.

4. Late Payment & Default

4.1 If the Client fails to make any installment payment within 10 calendar days after the due date, the entire remaining balance shall become immediately due and payable.

4.2 In the event of non-payment after the 10-day grace period, PHI BEAUTY LLC reserves the right to initiate legal action to recover the outstanding balance, including but not limited to filing a claim in court.

4.3 The Client agrees to be responsible for any legal fees, court costs, collection fees, and other expenses incurred by PHI BEAUTY LLC in enforcing this Agreement.

5. No Refund Policy

All payments made under this Agreement are non-refundable for any reason, including but not limited to cancellation, dissatisfaction, or failure to attend scheduled appointments.

6. Governing Law & Jurisdiction

This Agreement shall be governed by and construed in accordance with the laws of the State of Florida. The Client consents to the exclusive jurisdiction of the courts located in Florida for any disputes arising out of this Agreement.

7. Entire Agreement

This Agreement constitutes the entire agreement between the parties and supersedes all prior understandings, whether written or oral. Any amendments to this Agreement must be in writing and signed by both parties.

8. Client Acknowledgment

By signing below, the Client acknowledges that they have read, understood, and agree to all terms and conditions of this Agreement, including the requirement to provide a valid photo ID.
